Social Anxiety Screening Test

The following is a short questionnaire designed to help measure the severity of social anxiety symptoms.

The test involves answering a series of questions related to different examples of ways in which social anxiety can affect your feelings and behaviors. For each example, you’ll rate to what extent you identify with what is being described.

This test does not offer a diagnosis. Rather, it uses principles from the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition (DSM-5) to give a high-level indication of the severity of social anxiety symptoms you’re experiencing.

Based on your feelings over the past two weeks, how much would you say you identify with each of the following statements?

Question 1 / 10

Feeling intense fear or anxiety in social situations (for example, meeting new people, small talk, groups).

Question 2 / 10

Worrying that others will judge you negatively (for example, think you are awkward, boring, or incompetent).

Question 3 / 10

Avoiding social situations because of fear or anxiety.

Question 4 / 10

Enduring social situations but with strong distress (for example, you “push through” while feeling awful).

Question 5 / 10

Fear of being the centre of attention (for example, being watched, eating/drinking in front of others, speaking up).

Question 6 / 10

Physical anxiety symptoms in social situations (for example, racing heart, shaking, sweating, blushing, nausea).

Difficulty speaking clearly in social situations (for example, voice shakes, mind goes blank, can’t find words).
